FIRST TIME PATIENT

YOUR FIRST VISIT: 1 – 1.5 hrs 

Welcome to the office!

  1. Arrive about 10 minutes before your appointment time to fill out selected forms.
  2. Fill out patient history and relevant information forms. If this is a workman’s compensation or personal injury case, or you are using insurance, please have the important information ready to be photocopied and verified for benefits. (If you can have the forms faxed to you or downloaded, you can save about 20 minutes!) Knowing your benefits before scheduling will help you prepare you for your visit. The office manager will verify your benefits or information while you are filling out forms.
  3. Turn in the forms for processing.
  4. You will get a brief tour of the facility and when the conference room is ready, The office manager will take you there to watch a video about chiropractic care while the doctor reviews your information.
  5. After the video, the doctor will answer some preliminary questions before acquiring your written consent for taking the case history and performing examinations.
  6. From the history, the doctor will determine and perform problem-focused physical, orthopedic, and chiropractic exams. The radiological exam will be determined by those findings.
  7. After the exams, there may be time available for treatment. If not, you'll schedule an appointment to discuss the findings and adjustment schedule. You'll schedule the second visit at the front desk and pay for services before leaving.
YOUR SECOND VISIT: 30-45 minutes 
(If time is available or your condition warrants immediate care, this visit may be combined with the first visit)
  1. The office manager will take you to the Conference Room to watch the video outlining the doctor's findings from the exams.
  2. The doctor will discuss the findings and outline the patient management plan for care. Each patient's treatment plan and schedule may vary based on their improvements.
  3. You'll be taken to the adjusting room where the routine chiropractic exam will be done to review spinal misalignments, associated tissue spasm, and symptoms. Using a variety of different chiropractic techniques, the patient's spine, extremities, and other muscle and soft tissues may be adjusted to promote healing.
  4. Patient pays for services for the day and further appointments are scheduled.

Additional Visits:  5-15 minutes

  1. Arrive at the scheduled time or before.
  2. You will be taken to an adjustment room to be evaluated and adjusted.
  3. Patient pays for services or co-pay for the day.  Further appointments may be scheduled.

Becoming a patient is an organized process.  By conducting the business and paperwork in an organized manner, the doctor can spend more of the office visit with you and less with the paperwork that goes with your visit.
